- Applicants will be required to come in person and submit the following:
– Duly filled in application form – to be submitted in duplicate. Please ensure usage of the latest form which may be found here…
– Original Indian passport/Canadian passport/OCI Card, as applicable.
– No fee is charged for life certificate for pension purposes.
– IMPORTANT : In case the applicant is unable to come in person due to medical reasons, the bearer of the application will be required to set up a video call with the applicant for verification purposes.
